Overview

Sermorelin (GHRH 1-29) is the biologically active fragment of growth hormone-releasing hormone. It was the first GHRH analogue approved for use in GH-deficient children and is now widely used in anti-aging medicine for physiologic GH restoration. It stimulates the pituitary to produce its own GH rather than providing exogenous hormone.

How it works

Binds GHRH receptors on pituitary somatotrophs, triggering endogenous GH production and release in a pulsatile pattern. Preserves all normal feedback mechanisms (somatostatin, IGF-1 feedback). Over time, may actually improve pituitary GH capacity rather than suppressing it (unlike exogenous HGH).

Half-life and peak

Very short half-life of approximately 10–20 minutes due to rapid DPP-IV enzyme degradation. GH pulse occurs within 15–30 minutes of injection and lasts ~60 minutes. Typically dosed before bedtime to amplify the natural nocturnal GH surge.

What it is studied for

Pediatric GH deficiency, adult GH optimization, anti-aging protocols, preservation of pituitary function, sleep improvement, body composition.

Research status

FDA-approved for diagnostic use and pediatric GH deficiency (Geref). Used off-label in anti-aging medicine. Well-characterized safety profile. Manufacturing discontinued by Serono but compounding pharmacies produce it.
